• Shot Record, Last Physical & Dental Exam
• Driver’s License / ID
• Medicaid/CHIP/Private Insurance & SNAP
• 2020 Tax Return / W2
• Pay Stubs & Child Support (Previous 12 Months)
• Current Recipient TANF Letter
• Current Recipient SSI Letter
• Letters From Employers (Form Available)
• Social Security Benefits
